Basic Procedures

1. Once you have discussed your service requirements with our sales department toll-free 1-800-367-2432, new customers please read and sign our Service Agreement and Request for Services then, Fax to: 1-800-422-5711, or Mail to Employer’s InfoSource. You will be notified as soon as your account has been established.

2. To order background checks, simply fax your applicant's signed release form toll free to 800 422-5711. As a minimum, we require the applicant release form (Request, Authorization and Release of Background Information) completed and signed by your applicant(s). This form doubles as a service order when faxed or mailed to our offices. The portion of the employment application that you wish to have verified is also essential. Please include any special instructions or messages pertaining to a particular assignment in the box labeled "office use only." Also, please ensure complete and legible identifying information pertinent to your job applicant is submitted including full names, (Jr. Sr. II,) SSN, DL, addresses, zip codes, former employer's telephone etc.

3. Under the Federal Fair Credit Reporting Act (FCRA), you are required to give the subject of Consumer Report (applicants), a copy the “Summary of Consumer Rights Handout” at the time you present them with the required disclosures and release/authorization forms. Please print a copy of the Summary of Consumer Rights Handout and make sufficient copies for future use. Simply provide a copy to each applicant you are requesting any background verification service on.

4. California Employment Only: When requesting a consumer credit report for employment purposes, the subject of the credit report must read and sign the separate notification and authorization form required by the California Consumer Reporting Act. Applicants will indicate whether they request their own credit report mailed to their home address, free of charge directly by the credit bureau. Also, California law requires California employers to automatically provide a copy of the background report to the applicant.

5. Results of applicant background verifications are normally available within 2-4 business days depending upon the nature and scope of the services requested. Occasionally however, factors beyond our control will cause reporting delays. This includes criminal court record availability, court record retrieval policies, state MVR policies, school record verification policies and individual employer policies. Records may be archived, and employers may require written requests for employment verification. Naturally, Employer's InfoSource will pursue the most reliable and expeditious route to obtain the desired information. Also, we will accommodate your administrative and reporting requirements whenever possible. Completed reports can be faxed to your private or attended fax machine and or a verbal report will be issued via telephone.

6. Source document retrieval of court records, school transcripts etc. is available at our cost.

7. Standard terms of payment for services rendered is Net 30 days.

Our office hours are from 8:00 AM to 5:00 PM PST, Monday through Friday. We can receive your Fax transmissions 24 hours a day. Our Fax and voice mail is accessible only to authorized personnel in order to protect applicant privacy.
